On-Site Mental Health Services for Residents

At Health at Home Consultants, we provide compassionate mental health care to residents in assisted living and long-term care facilities. Our trained APRNs (Advanced Practice Registered Nurses) screen for conditions like anxiety, depression, or insomnia, offer basic education and support, and manage mental health medications, all within the facility setting.


Our goal is to ensure mental well-being is treated with the same priority as physical health.

Our Approach to Mental Health Care

Due to high demand, our dedicated mental health provider currently serves facility-based patients only. This ensures we can provide consistent and focused care where it’s needed most.


For patients seen in the home, our APRNs still receive support from our mental health provider behind the scenes, offering consultation, guidance, and recommendations as part of a collaborative care approach.

If a resident is experiencing a mental health emergency or crisis, please call 911 or direct them to the nearest emergency room. Our services are not meant for emergency treatment.

Medical Director Services for Skilled Nursing Facilities

Meet Dr. Zaid Fadul

Dr. Zaid Fadul is a licensed physician in all 50 states and serves as Medical Director for Health at Home Consultants. His background spans Family Medicine, Aerospace Medicine, Occupational Health, and Addiction Medicine.


As a former Chief Medical Officer and healthcare innovator, Dr. Fadul has led national-level care delivery systems. He specializes in integrating telehealth with traditional care to improve outcomes, reduce hospitalizations, and ensure that residents receive well-coordinated, quality care.

  • What if the provider quits? Would there be a back-up plan to continue services until a replacement is found?

    Yes, we have a contingency plan in place to ensure that services continue seamlessly in the event a provider leaves. Our team is structured to provide ongoing support, and we work proactively to identify coverage solutions while securing a replacement provider. Our goal is to avoid any disruption in care.

  • What is the next step?

    This includes your team identifying residents who may benefit from services, completing any necessary patient paperwork, signing the mental health services agreement, and scheduling initial visits. About 2 weeks before our first visit we like to do a meet & greet to go over expectations you may have for us as a provider in your building. Please take a look at residents you may feel would be appropriate for these services, and create a list. From that list, we can identify who we need to send our family letter to--or, we can provide that to you and you can send, whichever you prefer.

  • Do you have a letter we can send to our families to offer the services?

    Yes, we can provide a letter that introduces the services and informs families about what to expect. We can also customize the letter to align with your facility’s communication style.

  • Do we need a doctor referral to the services?

    A doctor’s referral is not required, but we do encourage collaboration with primary care providers to ensure comprehensive care. We find that if the PCP is aware ahead of time and gives their "blessing", things go smoothly.

  • Do you request health record from other mental health services for current patients wanting to switch or is this not necessary?

    While it is not always necessary, obtaining previous health records can be helpful for continuity of care. With appropriate consent, we can request records to better understand the patient’s history and treatment plan.

  • Are there any restrictions with skilled or hospice patients?

    No, there are no restrictions. We can provide mental health services to skilled nursing and hospice patients, working in collaboration with their care teams to ensure appropriate and supportive treatment.
